Service Coordinator

Location: Main Campus

Job Description:

  • Assess new members at admission for service needs & preferences
  • Develop service plan & change as needed periodically
  • Assist member in transition to ACV community & monitor regularly for effectiveness of service plan
  • Record and keep accurate all member data on a timely basis
  • Maintain contact with family members and other significant individual on behalf of the member
  • Maintain directory of agencies and service providers in the service area that would be a resource for member needs
  • Bachelor degree
  • Minimum of two years of relevant experience preferred
  • Valid Florida driver’s license required
